When you check for your state's living will form, please be sure to also find the form called the HIPPA Release Authority. In 1996 Health Insurance Portability and Accountability Act (HIPPA) has some provision that if you don't have a release in your living will, various medical authorities could theoretically refuse to give information about your medical condition and then your agent could not make an intelligent decision on what to do or not to do. Also, i recommed having a lot of copies of these documents so your decisions are know by everyone involved.
